1. Name of vendor (Store purchased from)
2. Transaction date/Purchase date
3. Item purchased (must show model #)

The hardware version can be found on the back/bottom of the device marked "H/W"
The Hardware version will be 1 letter followed by 1 number (e.g. A1)
